Hybrid follow-up care program: a combination of personal on-site care and digital support
We have developed a hybrid follow-up care program for our outpatients that combines the benefits of personal on-site care with the flexible options offered by digital services. This program enables you to continue your rehabilitation without interruption, as it combines regular therapy sessions at our facility with helpful digital content to use at home.
The hybrid structure gives you access to individually tailored treatment plans that include physiotherapy and occupational therapy exercises, along with additional content such as relaxation techniques and educational videos. Our goal is to offer you the best possible framework for your recovery based on this holistic care approach – effective, flexible and tailored to your needs.
«Reha to go» – Your hybrid follow-up care
Hybrid follow-up care is available in two different forms:
Optional exercise program: Use the Telereha app to access your optional exercise program at home. You complete the exercises yourself and do not receive any feedback from your therapist.
Accompanied exercise program: If you would like to receive feedback from your therapist during your hybrid follow-up care, you can request this as part of your outpatient treatment. Your therapist will receive an overview of your treatment activities and your feedback, allowing them to adapt the exercise program as required as part of your overall treatment.

In addition to our hybrid follow-up care for outpatients, we offer digital-only follow-up care following inpatient rehabilitation. More information can be found on the relevant institution’s website (Rehaklinik Zihlschlacht and Rehaklinik Dussnang).
«Reha to go» with Caspar
We work with Caspar Health to provide our hybrid and digital follow-up care services. The Caspar Health app offers a user-friendly, secure platform on which you can access all treatment content at your leisure and send direct feedback to your therapist team. Together with Caspar Health, we ensure that your follow-up care always matches the latest technical standards and treatment approaches.

It’s easy to access our «Reha to go» services from your smartphone or computer. Simply download the Caspar app from the App Store or use this link to access the hybrid aftercare program from your PC. To receive a Caspar ID, please contact your therapist.
